GP Surgery in Derby Placed Under Special Measures After the Care Quality Commission (CQC) Rate it as “Inadequate”

The CQC carried out an unannounced comprehensive inspection at The Osmaston Surgery based just outside of Derby City Centre on 5 June 2023. Overall, the practice was rated as “inadequate” and it has been placed under special measures.

On 16 August 2023, the CQC published their inspection report on The Osmaston Surgery. The report described the CQC’s judgement of the quality of care at the surgery. It was based on a combination of what the CQC found when they inspected the surgery, information from their ongoing monitoring of data about the surgery, and information given to them from the provider, patients, the public and other organisations.

Five key criterions were rated by the CQC. Four of these, namely, whether the services provided were safe, whether the services were effective, whether the services were responsive to people’s needs and whether the services were well-led were all rated as “inadequate.” The other criterion was whether services were caring and this was rated as “requires improvement.”

The most alarming findings made by the CQC were that the surgery did not ensure care and treatment was provided in a safe way to patients and that the surgery had not established effective systems and processes to ensure good governance in accordance with the fundamental standards of care. These constituted breaches of regulations.

A worrying example used by the CQC to evidence the GP surgery’s failings was that during the inspection, 236 pathology reports dating back to 10 May 2023 were awaiting filing. They found that 203 of these results had either not been viewed or had been assigned to others to action. Of these results, 77 were marked as abnormal. The CQC went on to say that when these were discussed with the partners, they were not aware of the outstanding results or those which had not been matched to patients. The CQC found that when tests were requested there was no follow-up arranged to ensure the patient was appropriately reviewed, including tests to exclude potential cancer diagnosis or diagnosis of diabetes.
